Basic Function:
Under the direction of the Chief Medical Officer, the Quality Improvement/Utilization Review/Improving Patient Care Registered Nurse Manager (QI/UR/IPC RN Manager) will help to establish and coordinate programs for quality improvement and utilization review in both CIR and LRJ health centers that will ensure that the best evidence-based medical practices are in place, in accordance with the IPC goals and SNHS policies.

Qualifications:
Bachelors of Science in Nursing with current NYS RN license. Requires 5 years RN Clinical experience. 5 years experience in a supervisory capacity. Must have 5+ years experience and or training in Quality assurance, quality improvement , utilization review, or patient- centered medical home/IPC. Knowledge of IHS standards and practices is a must.

Job Title: Health Information Manager

Basic Function:
Responsible for the overall operation of Health Information Management. Ensures the integrity of the electronic medical record, reviews documentation, oversees the coding process, and monitors HIPAA Privacy compliance throughout the health centers. Monitors the completion of referrals by obtaining reports from other healthcare entities. Oversees the release of information requests and monitors disclosures.

Qualifications:
Bachelors Degree in Health Information Management or related field must have current certification as a credentialed HIM professional RHIT or RHIA, with at least three years experience in a health information department and must have at least three years supervisory experience. Must possess thorough knowledge of medical terminology, coding, documentation, medico-legal processes, anatomy and physiology, HIPAA and Privacy Act of 1974, and release of information. Must have two years experience with electronic medical record systems. Possess excellent communication skills, both oral and written. Possess thorough knowledge and experience of computer application software including electronic medical record programs and Microsoft Word, Excel, PowerPoints, and Publisher. Background check may also be required.

Job Title: Diabetes RN Case Manager/CDE(Certified Diabetes Educator)
Rate: Grade 10
Location: Irving

Basic Function:
The Diabetes RN Case Manager/CDE will be primarily responsible for tracking persons with diabetes, providing individualized case management, group education and insuring health team collaboration for the purpose of improving patient outcomes. The RN Case Manager/CDE will perform the primary functions of a Registered Nurse in assessing, planning, implementing and evaluating the care of patients with diabetes, pre-diabetes and gestational diabetes. They will also provide diabetes and pre-diabetes education using the established guidelines from Indian Health Service, and American Association of Diabetes Educators Organizations and SNHS Medical Director. This employee will function according to NYS Nursing Law and I.H.S. standards of care for patients with diabetes, pre-diabetes and gestational diabetes. Another component of the RN/CDE aspect of this position is assuring that all attempts are made to holistically treat diabetes, pre-diabetes and gestational diabetes. Other duties include assisting with the planning and implementation of community-based diabetes activities and education programs.

Qualifications:
NYS Registered Nurse, (Bachelors degree preferred), verifiable Certified Diabetes Educator status. Candidate must have at least 2 years RN experience in diabetes care, treatment and education at the community level.

Job Title: Clinical Services Supervisor
Rate: Grade 13
Location: Cattaraugus & Allegany

Basic Function:
The Clinical Services Supervisor is responsible for daily service integration and quality of clinical and case management services provided by the Behavioral Health department. The incumbent will actively support and implement a strengths-based, culturally competent, client-centered approach to working with eligible individuals. Provides clinical and administrative supervision of behavioral health treatment providers and behavioral health case managers as well as clinical supervision/consultation to the substance use disorders and transition care management teams. Reports directly to the Director of Behavioral Health.

Qualifications:
Masters Degree in Social Work or related field from an accredited school. Must have current unrestricted New York State licensure as a Licensed Clinical Social Worker (LCSW) with R privilege (or eligible) or Licensed Clinical Psychologist. A minimum of five (5) years experience in a counseling capacity is mandatory. Must have at least three (3) years supervisory experience with clinical supervision. Highly organized with the ability to understand and provide feedback on budgetary expenditures. Ability to work with diverse staff. Job Title: Insurance & Reimbursement Specialist

Basic Function:
Under the direction of the Business Office Manager, the incumbent is responsible for reviewing of reimbursements from insurance plans, determine denial classifications, develop justifications for making billing changes, and enroll new physician and professional staffing with all third party payers.

Qualifications:
High school graduate and Accredited Record Technician (A.R.T.) with three years experience in reimbursement, coding, and clerical work in hospital or medical office setting. Knowledge of coding policies and procedures preferable. Computer skills and good communication skills both oral and written are essential. Must have training in medical terminology.
