The Cattaraugus Community Health and Wellness Center is located
on the grounds of the Cattaraugus Indian Reservation Health
Center in Irving, New York. This center provides services for
patients located on our Cattaraugus territory and in the
surrounding areas. |

| Our focus is to promote the
physical mental and social health of our community by offering
education, support systems and group based activities. |
The units located in this facility work to promote
fitness, healthy eating, mental health, substance abuse awareness,
family unity, diabetic education and the prevention of numerous
health problems.

The units we have located here are Behavioral Health Services,
Health, Outreach, Prevention and Education (HOPE), Child and Family
Services and both our Diabetes and Diabetes Prevention programs.
This building , which was constructed in 2005, was a wonderful
addition to our services here on the Cattaraugus territory. The new
space it provides gave us room to expand the aforementioned units
and gave way to extended areas for group activities and exercise
programs. |

| In this facility we
have kitchen areas available for healthy cooking classes and
diabetic friendly cooking demonstrations. |
| These areas
which are located off of group meeting rooms have
proven to be perfect way of promoting healthy life
style changes which help the people of our communities control health problems such as hypertension, obesity, diabetes and high cholesterol. We
have found that providing instruction and giving
our patients a chance to taste food that is better for
them, makes way for better results and greater change. |

The exercise room at CHWC is filled with a variety of exercise equipment and
is staffed by two full time exercise specialists. Our
Exercise Specialists guide our patients through
their prescribed exercise program, helping them with motivational skills, |

proper exercise instruction and goal objectives
that best fit their needs.
Our exercise program is available to our patients
by prescription only, insuring that the patient is
healthy enough to withstand physical exercise
and also ensuring that the patient stays within
the limits set forth by their provider. We have achieved great change with
this program and
strive to continue to help our community stay
fit and healthy. |

| At the CHWC we also host many community events. We organize activities for
the annual Community Health Fair, put on bike rodeos, host numerous
walking programs, hold our annual “Turkey Trot” event and much more. |
| We also host many self
improvement meetings, discussion forums, parental
education courses as well as many SNHD departmental meetings and much more. |
|
