The Cattaraugus Indian Reservation Health Center is located on
the Cattaraugus Territory in the town of Irving. This facility
offers a wide range of medical care to its surrounding
communities and also serves as the primary Health Center for
patients of the Buffalo Program Office. |

| Here at the CIRHC we provide medical services as follows:
preventive care, general examinations, check-ups, radiology
services, lab services, prenatal care, pediatrics, well child care,
obstetrics, gynecology, chronic disease monitoring, pharmacy,
podiatry, audiology, optical services, orthotics and dental. |

| Our priority is to provide quality health care for
our patients while emphasizing the focus on prevention activities.
In an ever changing environment where health care needs continue to
develop, we persist in our mission to provide a comprehensive range
of health services that address the holistic wellbeing of each
individual. At the Cattaraugus Indian Reservation Health Center (CIRHC),
located in Irving, NY, we provide the following services:

MEDICAL
We currently have a Family Practice Physician and an Internal Medicine
Physician who are focused on patient care throughout the week at our health
center. These Physicians also supervise and collaborate with our mid-level
medical staff which currently consists of a Family Nurse Practitioner and
two Physician Assistants.Additionally, our unit is staffed by a number of
highly qualified nursing staff both Registered Nurses(RN) and Licensed
Practical Nurses (LPN) alike.
In addition, we have Community Health Represenatives (CHR) to provide
some patient care and education on a limited basis to those who may be
homebound. Finally, this unit is assisted and supported by a medical
administrative assistant and a receptionist that provide necessary support
services for the unit to function smoothly. Services include preven- tive
care, acute care and chronic disease monitoring. Em- phasis is placed on
excellent pre-natal care, well-child checks and diabetes related care. On a
consultant basis we provide obstetrics & gynecology, pediatric care,
podiatry & orthotics, audiology services, and ultrasound.
Within this unit we also have a laboratory, basic radiology equipment,
and standard examination/treatment rooms. |

DENTAL UNIT
The CIRHC center operates a fully operational dental clinic where we provide
general dentistry & dental hygiene, pediatric dentistry and oral surgery.
On staff in the dental unit are two full-time dentists, two full-time dental
hygienists, dental assistants and a dental receptionist. On a consultant
basis we provide the services of a pediatric dentist and an oral surgeon for
patients of CIRHC, as well as those referred by the Lionel R. John Health
Center.
This dental unit has upgraded numerous items within the facility from
purchasing new state-of-the art dental tools to replacing damaged/dated
dental chairs to better accommodate patient comfort. |

OPTICAL UNIT
Optical services are made available by a full-time optician, an optometrist
and an ophthalmologist. In addition, the professional staff of the unit are
supported by a full-time ophthalmic assistant.Services include routine
eye examinations, screenings, and monitoring/treatment of conditions such as
glaucoma, diabetic retinopathy and cataracts. This unit is outfitted with
some state-of-the-art equipment used to pre-screen and assist in diagnosis
of conditions.
Products available include eye glass frames in a wide pricing options
from free to partial or complete out-of-pocket expense. In addition, contact
lens fittings are conducted and lenses, as well as supplies are available as
an out-of-pocket expense to our patients. |

PHARMACY
Currently this unit is staffed by a supervising pharmacist that oversees the
smooth operation of this all important unit including: four licensed
pharmacists, a certified pharmacy technician and an additional pharmacy
technician.Service include the accurate filling and dispensing of
medications per doctors orders, education and counseling for patients
regarding medication safety, interactions and possible side effects. |

HEALTH INFORMATION MANAGEMENT
This often overlooked unit is the central information processing point for
patient information. The services are provided are coordinated by a health
information manager, senior health information assistant and three health
information assistants. In October of 2006, the CIRHC changed over from a
solely paper charting system to an electronic medical record. This shift
created challenges and change within this unit.
The staff within this unit serves both patients requesting personal chart
information, as well as medical personnel who may require patient
information to create treatment plans etc. This unit is charged with
ensuring that the HIPAA standards are adhered to throughout our health
center and is continually moving us forward toward quality improvement with
regard to health information maintenance. |

ADMINISTRATION
The Health Services Administrator, two administrative assistants, and a
receptionist are responsible for the general office management issues of the
health center. This staff is also responsible for assisting patients with
the initial paperwork necessary to become eligible for services. They review
documents such as proof of address and tribal affiliation and conduct
eligibility interviews to inform patients about the services we provide for
them depending on their level of eligibility.This unit provides the
switchboard and waiting area coverage necessary to assist patients with
their visit to the health center. In addition this unit responds to
comments/complaints/compliments with regard to services provided at the
CIRHC.
