| We offer both our Diabetes Core and Diabetes
Prevention Programs on both territories. These programs, which
utilize our Cattaraugus Community Health and Wellness Center as
a home base, are a vital addition to our communities. Diabetes
manifests itself at high rates among Native people and we here
at the SNHD strive to combat and to care for this disease that
affects so many of our lives. |

This productive program has been benefiting our people since its
beginning in 1998. The goals for this program include; improving the
care of diabetes patients; decreasing the incidence of complications
and the prevention of diabetes onset. |
| Diabetes education to both medical staff and
patients has been very influential in decreasing the average blood
sugar levels and the rate of diabetes complications within the
Seneca people. This program also supports many different
sports/exercise events and community projects that promote increased
physical activity. We offer group classes at both territories, one
on one diabetes education and diabetic counseling to the community. |

| The Special Diabetes Prevention Program aims to
prevent diabetes in Native Americans. Before a person becomes
diabetic, they almost always have a condition known as
"pre-diabetes"in which blood glucose levels are higher than normal
but not yet high enough to be deemed diabetic. |
| Even pre-diabetics can reduce their risk of developing diabetes
and that is the task of the DPP. The Diabetes Prevention Program
works with you both in a classroom setting and on a one on one basis
to help patients avoid developing diabetes. The goal of this program
is to promote exercise, weight loss and eating healthy. Our staff is
there to support and guide our participants to success. |
