The vision of the SNHS Community Health Services is to provide
holistic health care, effective medical case management and
advocacy for community members of all abilities in order to
assist them to maintain regain or enhance their level of social
and physical functioning, when their ability to function is
affected by actual or potential illness, injury or disability. |

What the H.O.P.E Unit can do for you: |
- Provide health education, WIC, medical advocacy, and
transportation to medical appointments.
- Assist in coordination of outreach services for community
members through the SNHS and/or ancillary agencies.
- Assess community health care needs, identify individuals and
groups at risk, as well as other factors that impact health
care. Community input is a valued resource utilized for
program development.
- Plan, develop and implement health education programs
consistent with the existing needs of the community, with
emphasis on health promotion and disease prevention.
