Our HOPE unit is also based out of the Cattaraugus Community
Health and Wellness Center and also has a program available for
our Lionel R. John Health Center on our Allegany Territory. The
HOPE units main focus is community outreach that aims to better
the health and wellness of our people. |

Here in the HOPE unit we reach out to the community members in numerous
ways. Our units staff consists of LPN’s, a Registered Dietitian, Health
Educators, Nutritionists, an in-house WIC Coordinator and Medical Social
Workers.
We provide health, nutrition and fitness education along with case
management and outreach service coordination throughout the health
department and/or ancillary agencies. Our staff makes visits to hospitalized
patients in order to better evaluate post discharge care needs and also
makes visits to patients after discharge. We provide inter-departmental
referrals and assessment of community health care based on existing needs
and environmental factors. Our department plans and implements with
continuing emphasis on health promotion, disease prevention and case
management that supports optimum standards based on our communities. |
