Job Title: Board Certified/Eligible Family Practice & Internal Medicine Physicians
Rate: Negotiable
Location: Seneca Nation Health Department

Basic Function:
Under the direction of the Chief Medical Officer, the incumbent is responsible for the overall direction of the professional medical staff including consultants. Also provides primary medical consultation, advice, and opinions for overall Department medical operations. Reports directly to the Chief Medical Officer.

Qualifications:
Graduation from an accredited medical school, DO or MD, with board certification in area of specialization, and current license to practice medicine in the State of New York. Must have a minimum of three years experience in a clinical setting with knowledge of quality assurance/continuous quality improvement, risk management and peer review practices. Must pass a pre-employment drug screen in accordance with the Drug, Alcohol and Controlled Substance Abuse policy. Background check may also be required.

Job Title: Medical Social Worker
Rate: 20.09
Location: CIRHC/CHWC (Irving, NY)

Basic Function:
Conducts psychosocial assessment of patients and families upon patient self-referral, or CIRHC referral to medical social work. When assessment determines the need, provides crisis intervention or counsels patient/family on a short-term basis. Prioritizes needs, offers options and provides information and resources. Promotes self-help empowering patients and families to seek and maintain further treatment or assistance. Participates at micro, mezzo, and macro levels to ensure that an optimal level of care is provided to the population served by the CIRHC.

Qualifications:
QUALIFICATIONS: Masters degree from an accredited school of social work. Experience and credentials relevant to the health care industry. SPECIAL REQUIREMENTS: Member of the National Association of Social Workers (NASW) and adheres to the NASW’s ethics, values, continuing education requirements, and practice guidelines (specific to medical social work). PERSONAL ATTRIBUTES: - Knowledge of social work practices. - Understanding of public health systems. - Ability to perform culturally competent medical social work practice - Ability to communicate effectively orally and electronically - Skills in analysis of situations - Ability to develop collaborative relationships with other professionals - Ability to develop effective working agreements with other agencies. - Capable of functioning independently with a minimum of supervision.

Job Title: 1 FT Physical Therapist

Basic Function:
By physician prescription, appropriately utilizes skill to evaluate and design programs to improve the condition of medically referred patients to maximize healing, relief of pain and to restore and improve physical capabilities and performance.

Qualifications:
Bachelor of Science from approved School of Physical Therapy by the APTA or higher academic degree (Masters) or minimal level of education changes. If a Doctorate of Physical Therapy becomes the minimal education degree to practice Physical Therapy in NYS or the USA then that level is the SNHD educational minimal requirement along with appropriate registration and certification (this would apply to new graduates). If new graduate, eligible for New York State licensure exam, must have applied for temporary license. Must be compassionate, patient, and understanding. Job Title: 1 FT IT Software Database Report Writer
Rate: $18.06
Location: LRJHC (Salamanca, NY)

Basic Function:
Provides support to SNHD staff by collecting, analyzing and maintaining data, and reporting results. Meets with end-users to analyze data requirements for reporting, translating requirements into data modeling for report content. Utilizes software to generate metrics. Experience with Crystal reports. Use of complex expressions to calculate, group, filter, parameterize, optimize and format contents of reports. Basic understanding of healthcare practices, data and reporting requirements.

Qualifications:
Bachelor degree in Computer Science or related field preferred. 2 plus years of experience required in: Excel, Access, SQL Server Databases. Strong SQL query knowledge is a must. Experience using a database report writer is required. Thorough understanding of database driven report design, data modeling and data mining required. Works with and communicates effectively with staff at all levels of the organization. Must be able to work independently and as part of a team. Basic understanding of healthcare practices, data and reporting requirements.
