Transportation

Priority is given to patients in need of dialysis or chemotherapy; and is provided once other resources have been determined ineligible. Patients who have Medicaid will be required to apply for Medicaid transportation. Transportation is available to medical appointments at SNHS. Please be aware that 72 hours advance notice is required. It is the patient/client's responsibility to request transportation.
